Skilled Nursing Facilities

The Skilled Nursing Facilities (SNF) Dept is responsible for the case management of Kaiser members in contracted SNF facilities. The skilled care must be ordered and the patient followed by a TPMG Physician. The care must be medically necessary and above the level of custodial care. This care would include skilled nursing by a licensed nurse on a daily basis or needing physical, occupational or speech therapy on a daily basis with a minimum of 5 days per week. Ongoing therapy in the SNF is based upon the patient’s ability to demonstrate significant, predictable and functional progress.
