3 things you need to know about Form 1095-B

Under the Affordable Care Act (ACA), you must show proof of health coverage to the IRS or pay a penalty. Kaiser Permanente will send you Form 1095-B on or after January 6, 2017, so you can prove you’re covered.

 

Form 1095-B provides information about your health coverage, including who was covered and when the coverage was in effect. Kaiser will send the form to the IRS on your behalf and send you a copy for your records.

 

So, what else do you need to know about Form 1095-B? Here are 3 key things:

 

1. Check to make sure all of your information is correct.

 

This includes your name, address, Social Security number, date of birth, coverage period for you or your dependents, and all other personal information on this form.

 

2. If any information is wrong, let us know as soon as possible.

 

Go here for the number to call to fix any errors or ask questions. Be sure to do this as soon as you find any mistakes.

 

If you’re correcting your Social Security number, date of birth, or coverage period, you should get your new form within 30 days. If you’re making changes to your name or address, you won’t get a new 1095 form, but we’ll send your updated information to the IRS.

 

3. If your information is correct, don’t submit it with your other tax forms.

 

Be sure to bring Form 1095-B with the rest of your tax forms to give to the person who files your taxes. If you file your own taxes, don’t submit Form 1095-B with your other tax forms, but instead keep it for your records.
