Continuing Care

The team of Fresno’s Continuing Care Department includes physicians, nurse practitioners, and patient care coordinators. We are a diverse group that reflects the population of our service area. We work together to care for our members in the Fresno community. Patient care coordinators work with patients and their families to help patients make the transition from the hospital to a skilled nursing facility and from skilled nursing to home. Continuing care physicians and nurse practitioners make regular visits to the skilled nursing facilities to provide medical care for our members who are admitted for rehabilitation after surgery or a medical illness. We also provide medical care for our long-term residents in these facilities. Our goal is to promote and maintain the health of all our patients in skilled nursing facilities and to assist them to regain the highest level of function. We also work closely with local hospices when patients need end-of-life care.
