Look for the "Make Your Wishes Known" booth at the farmers' market in front of the Parkview building.

Make your health care wishes known by completing an “Advance Directive” form. An Advance Directive is a legal document that makes sure you will receive the care you want, even if you become too ill to make future decisions about your care.

Doctors and staff will be on hand at our farmers’ market “Wish Booth” to answer your questions. A notary will also be available to notarize your form if you bring valid photo ID and $5.
