Home Health Care

The mission of the Home Health department is to provide a transition from acute illness to independence at home by providing licensed nursing, rehabilitation and social services to Health Plan members within the KP service area and to those who meet the following admission criteria:

• KP physician determines treatment to be skilled and medically necessary and directs patient care
• Receives care in safe/appropriate setting to meet health care needs
• Illness/Injury results in confinement to home
• Patient/Caregivers participate in care plan/treatment goals
