Our Department

Welcome to South Sacramento Chronic Conditions Management’s Nutrition Team home page. We serve Kaiser Permanente members who need medical nutrition therapy or specialized nutrition advice. Our staff members are registered dietitians who work in close collaboration with the Departments of Internal Medicine, Pediatrics, Pulmonary, Gastrointestinal, and Health Education to provide the best nutritional approach for our clients.

How to Get Care

All classes and support groups are free to members and their families. Members can sign up for classes by calling the Nutrition Advice Line 916-688-2457.

Diabetes Nutrition – Carbohydrate Counting! (Also in Elk Grove)

Get tips on planning meals, counting carbohydrates, reading food labels and review the four cornerstones of diabetes control.

Meal Planning for Diabetes – Must Have Taken Diabetes Nutrition

Enhance your carbohydrate counting skills! We’ll discuss meal planning, label reading and dining out.

Diabetes Support Group – Join Us! Registration Is Not Required.

This group will include guest speakers and dietitians specializing in nutrition, exercise, medications, reducing complications, and emotional well-being. Come join us every month at South Sacramento Wyndham Building, 1st Tuesday of the Month 5 to 6:30 p.m. or join us at South Sacramento Kaiser, 6600 Bruceville Road, 2nd floor, Building 1, Room#214 (near Station E) on the 2nd Wednesday of the Month at 10 a.m. to 12 noon.

Heart Health – Healthy Foods, Happy Hearts! (Also in Elk Grove)

If you have high cholesterol or high triglycerides you can learn heart healthy ways to reduce your risk for heart disease. In this class, you’ll learn how a healthy, nutritious diet can lower cholesterol levels. We’ll also cover low fat cooking techniques, dining out and a review of complementary therapies in heart disease treatment.

Label Reading – Cruise on the Sea of Healthy Eating! (Also in Elk Grove)

Do you have difficulty understanding how to read those tricky food labels? This class will help consumers understand how to interpret food labels in order to make the healthiest choices when shopping for food. General healthy eating guidelines and portion sizes will also be discussed.

Hypertension Nutrition – Go for the DASH!

If you have high blood pressure, this class can teach you how to incorporate the DASH (Dietary Approaches to Stop Hypertension) dietary strategies into your everyday life. You’ll discover how to reduce sodium, read food labels and maintain a healthy weight.

Managing Your Weight – The Shape of Good Eating!

Learn how making healthy lifestyle choices, including regular exercise, proper nutrition, and a positive attitude, can help you lose weight and keep it off.

Weight Wise – Where You Are the Star!

This 4 session series is designed to help you take control of your weight by cultivating mind, body and lifestyle. Class materials $20. No fee for class.

Weight Management Support Group – Join Us! Registration is Not Required.

This group will include guest speakers and dietitians specializing in nutrition, exercise, behavioral strategies, reducing complications, and emotional well-being. Come join us every month at South Sacramento Wyndham Building, 4th Tuesday of the Month, 5 to 6:30 p.m.

Get Moving – Let’s Go Everybody!

Want to become more physically active? Join us to explore ways to increase motivation and overcome barriers.

For Families we have three interactive classes to choose from.

  • Feeding Your Baby and Toddler
  • Healthy Eating, Active Living for Kids and Families
  • Healthy Eating, Active Living for Teens and Families

In all these family classes we cover basic nutrition, strategies to improve our dietary choices, overcoming barriers, positive body image, and physical activity. Parents and their children are encouraged to attend.
