Kaiser Permanente Santa Rosa Medical Center has been a Joint Commission certified Primary Stroke Center since 2009. We are proud to be an integral part of the stroke team, delivering high quality care to patients with acute strokes. Kaiser Permanente Santa Rosa honored with Quality Achievement Award for Stoke Care.
Your First Visit
New or first time patients are seen by referral from your personal physician. You may be asked to fill out a patient questionnaire. If you receive one, please bring it with you to the visit. While we are able to review your laboratory reports, medications filled at the Kaiser Permanente pharmacy and recent imaging studies on our computers, we do not have access to non-Kaiser Permanente medical systems. For this reason, please make sure we receive copies of all records or x-rays taken at other facilities that pertain to your current problem. It is often helpful to bring a list of specific questions and concerns you may have to each visit.
