Santa Clara 1st floor Laboratory (Dept 168)
Monday through Friday, 6:30 a.m. to 7 p.m.
Saturday and Sunday, 7 a.m. to 4 p.m.
Santa Clara 2nd floor Laboratory
Monday through Friday, 8:30 a.m. to 4:30 p.m.
Mountain View Laboratory
Monday through Friday, 8:30 a.m to 5:30 p.m.
*To help ensure patient safety with adequate physical distancing and reduce wait times, we’re now offering appointments for lab visits. When your doctor orders your lab tests, you can use kp.org or the mobile app to schedule a lab appointment.

Our Health Education department is temporarily closed until further notice. We’re available by phone at 408-851-3800 Weekdays between 8:30 a.m. to 4:30 p.m.

Our Member Services offices are temporarily closed. Call 1-800-464-4000 or visit kp.org for questions about your coverage, benefits, co-pays, or premiums, as well as complaints/grievances, and outside claims.
Member Outreach / Physician Selection
Our Member Outreach/Physician Selection department is temporarily closed. Call 1-888-466-1800 or visit kp.org/mydoctor to select a personal physician or for information about our practitioners.
Release of Medical Information (ROMI)
