Welcome to Advanced Heart Failure / Heart Transplant department.
We offer advanced heart failure consultation and management, cardiac transplant evaluation, post-transplant management, left ventricular assist device evaluation and access to a variety of clinical research trials. Following intensive consultation, education and evaluation, the majority of heart failure patients referred to our program, ‘graduate’ back to the care of their local general cardiologist and primary care provider once their symptoms are optimally controlled. We provide ongoing continuity care for patients on the heart transplant waiting list and cardiac transplant recipients.
We treat your heart problem as well as help you and your family understand and cope with your condition in the best way possible for you. That is why we have an excellent multidisciplinary team composed of physicians, nurses, clerical staff, and allied health professionals to help take care of you. It is this unique team approach that allows us to provide you with high quality care for you and your heart.
