The Kaiser Permanente San Rafael Department of Outpatient Social Work provides resources and assistance to address problems impacting our member’s individual health and wellness. We have compiled some commonly requested information and made it accessible to members here on our website.

The Outpatient Social Work team works in collaboration with your primary care physician, who remains in charge of your care. For additional Social Services support, please contact your primary care physician for a referral.
