The goal of the Kaiser Permanente San Jose Memory Clinic is to provide our members who have a diagnosis of dementia with the services, resources and support necessary to maintain successful quality of life.

A diagnosis of dementia (including Alzheimer’s disease as well as other types of dementia) can bring feelings of anxiety, fear, and a sense of loss of control to the patient as well as the caregiver and/or family members. The interdisciplinary team of the Memory Clinic is here to help you navigate the steps that follow a diagnosis, answer your questions, and provide you with the resources and support necessary to help you know that you do not have to deal with this alone.

How Do I Contact the Memory Clinic?

Your primary care physician or nurse practitioner will refer you to the Memory Clinic. We will review the referral and a case manager will contact you to schedule you and/or your family members for a class or office visit.

The case manager acts as your partner, to provide the services and support that will help you lead as full and productive a life as possible. The case manager remains your point of contact in the Memory Clinic.

The Memory Clinic team works in collaboration with your primary care physician, who remains in charge of your care.

Alzheimer’s Support Groups

Our Support Groups offer facilitated support and discussion for those who are caring for someone with dementia.

Caregiver Support Group

Sessions: Second Monday of every month, from 5:30 to 7 p.m.
Fee: No fee
