The Department of Adult and Family Medicine at Kaiser Permanente San Jose Medical Center is dedicated to providing quality medical care and service to our members

Our Mission Is…

  • Personalized care in a safe environment
  • Timely, courteous and professional service
  • Guidance to our patients in using health care services
  • Education and support to help patients achieve a healthy lifestyle
  • Quality care in a cost effective manner
  • Recognizing and respecting cultural diversity within our patient population The Department Of Adult and Family Medicine is organized into a module system for better continuity of care for patients. We encourage all members to have a personal physician or nurse practitioner. A personal provider works closely with his/her associate providers within the module. This assures that if the primary care provider is not available, one of his/her associates is available for any problems or issues, and information will be conveyed to the personal provider. This also assures that an individual’s medical care will be provided by a small group of providers, who will gain familiarity with their patients, in order to provide excellent care.
