  • Recognizing and respecting cultural diversity within our patient population The Department Of Adult and Family Medicine is organized into a module system for better continuity of care for patients. We encourage all members to have a personal physician or nurse practitioner. A personal provider works closely with his/her associate providers within the module. This assures that if the primary care provider is not available, one of his/her associates is available for any problems or issues, and information will be conveyed to the personal provider. This also assures that an individual’s medical care will be provided by a small group of providers, who will gain familiarity with their patients, in order to provide excellent care.


Scheduling and canceling Medicine appointments

Appointments may be made from your personal provider’s home page at My Doctor Online by using the “Manage & Schedule Appointments” link at the top left of the home page.

Appointments may also be made by calling 408-362-4791. You can either speak to an appointment clerk, or use our Automated Appointment Services for primary care.

Cancellations may be performed from your personal provider’s home page at My Doctor Online using the “Manage & Schedule Appointments” link or by calling 408-362-4791.

Get to know your doctors and browse recommended information and helpful online tools on My Doctor Online. From your doctor’s home page, you can also:

  • manage (schedule, view, cancel) appointments
  • refill prescriptions
  • check test results
  • email your doctor
  • view preventive health reminders and past visit information.
