Memory Clinic

Elder person enjoying a day at the park with her family member sitting at a stone tableThe Memory Clinic is a multi-disciplinary program designed to help provide San Francisco Kaiser Permanente members with a comprehensive assessment of their memory problems and link them with needed resources. We understand how memory loss can affect your life and the lives of loved ones. The Memory Clinic team works together to develop a personal evidence-based treatment plan that addresses the needs of patients as well as their family and caregivers. Our goal is to provide our members with education, supportive services, resources, and guidance. If either you or a loved one is experiencing symptoms of memory loss, our team can help.

Your primary care physician or nurse practitioner will need to refer you to the Memory Clinic. We will review the referral and contact you to make the appointment.
