Coaching and Counseling
Clinical Health Educator
Make an in-person, video, or phone appointment with a Clinical Health Educator. Our Clinical Health Educators are available to provide individual education and counseling for a range of health concerns. Education topics include weight management, general nutrition, exercise, smoking cessation, stress management, cholesterol, hypertension, and prediabetes care.
Available in English, Spanish, and Cantonese for members 18 and older. No fee. To schedule, call 415-833-3450.
Personal Health Coach Program
The Personal Health Coach Program is offered at Kaiser Permanente San Francisco Medical Center’s Health Education Department. We provide one on one coaching to help optimize your health and wellness goals through nutrition and fitness coaching by a Registered Dietitian and a Physical Therapist / Certified Personal Trainer.
Available in English and other languages via interpreter for members 18 and older. Costs vary. Open to the community.

Lifestyle Medicine Physician Consultation
The Lifestyle Medicine Physician Consultation will provide personalized, whole person care in a longer office visit that will include:
- Measurement of cardiometabolic vital signs
- Comprehensive dietary assessment, review of medical history and lifestyle
- Co-designing a plan to help you reach your health goals using evidence-based, personalized and data-informed solutions.
- Exclusive discounts on meal delivery (optional)

Culinary Medicine Physician Coaching
The Culinary Medicine Physician Coaching is a unique service to improve your health where it begins, in the kitchen. All initial consultations with be 60 minutes and include:
- Counseling on meal planning, grocery shopping, nutrition label reading and mindful eating.
- Culinary medicine prescription with recipes customized for your food preferences, dietary restrictions, health conditions, and cooking skills.
- Exclusive discounts on meal delivery (optional)
Follow up appointments may include.
- Virtual kitchens consult with advice on pantry stocking and equipment for your home kitchen (30 minutes)
- Private cooking instruction (60 minute minimum, in person or virtual)

Registered Dietitians
Registered dietitians specialize in pediatric, adolescent, and adult nutrition care and are dedicated to helping members prevent disease and improve their overall health through nutrition therapy. We offer medical nutrition therapy and individualized nutrition advice for specific medical conditions, such as heart disease, cancer, renal disease, liver disease, diabetes, pregnancy nutrition, amongst others. A referral from a health care provider is required to schedule an appointment.
