Adult and Family Medicine
The Department of Adult and Family Medicine has a longstanding tradition of excellence in comprehensive and coordinated primary care, and serves a culturally and socioeconomically diverse patient population in the Bay Area. Our Adult Medicine clinicians provide expert coordinated primary care for members over 18 years of age, while our Family Medicine practitioners offer comprehensive care to members of all ages.
We provide adult and family primary care services with a team of professionals including your Primary Care Physician or Nurse Practitioner (NP). Our innovative model of providing individualized medical care in the outpatient setting consists of physicians and nurse practitioners working in concert with allied health care professionals to provide a multidisciplinary approach to patient care. As primary care doctors and NPs, we place a strong emphasis on preventive care, health education, and appropriate disease management and medical interventions. Your relationship with your Primary Care Doctor or NP is designed to be long-term and ongoing, in order to provide continuity of medical care and allow you to make informed decisions about your health care needs.
We have eight Adult and Family Medicine teams, each consisting of physicians, nurse practitioners, case managers, clinical health educators, medical assistants and other health professionals who work together to provide routine care and address complex medical conditions. Two of our teams focus on serving our members who prefer to speak in Cantonese or Mandarin, and those who prefer to speak in Spanish. We also have a team of doctors and NPs experienced in caring for our members with HIV. We coordinate care with expert specialists in the Departments of Oncology, Hematology, Nephrology, Gastroenterology, Pulmonology, Rheumatology, Infectious Diseases, Endocrinology, Travel Medicine, Community Based Medicine and Pediatrics, working closely as a unique group practice. The services of our specialists can be accessed by a referral from your primary care doctor or nurse practitioner.
