If you do not consent to this form of care, if you have any questions or if you prefer to disenroll, please call the Department at 916-973-5300.
For a successful and confidential class experience for yourself and others, you must have a quiet, private location where you will not be interrupted and others (who are not participating in the class) cannot overhear the discussion of the class. Along with Kaiser Permanente’s commitment to foster respect for all in the class, each participant also has a responsibility to respect and protect each other’s privacy. Please share useful general information outside of the Class, but what you hear and learn about individual Class members should not be shared. This MS Teams service includes a feature that allows for audio to be heard and documents and other materials exchanged to be viewed during the session. By joining this session, you automatically consent to participate. If you do not consent to participate, discuss your concerns with your individual provider prior to the start of the meeting or do not join the session.
