Memory Clinic

The Memory Clinic is an interdisciplinary program designed to help provide Napa-Solano Kaiser Permanente members with a comprehensive assessment of their memory problems and link them with needed resources. The Memory Clinic team understands how memory loss can affect your life and the lives of loved ones. We provide a comprehensive evaluation to identify all possible causes of a memory problem. The team works together to develop a personal evidence-based treatment plan that addresses not only the needs of patients but also their family and caregivers. Our goal is to provide our members with education, services, resources, support and guidance every step of the way, to help improve the quality of their lives. If either you or a loved one is experiencing symptoms of memory loss, our team can help.

How to Get Care

Your primary care physician or nurse practitioner will refer you to the Memory Clinic. We will review the referral and contact you to schedule you and/or your family members for a class or office visit.
