How to Apply to Kaiser Permanente San Leandro CPE

• Download a standard ACPE Application from acpe.edu.
• Complete the application in its entirety by answering the essay questions fully and by filling out the face sheet completely. Completed application should be about 7-10 pages typed. For continuing students, also include a recent verbatim and copies of student and supervisor final evaluations for any previous ACPE units.
• Completed Application and Authorization Form for release of student information, and application fee sent to: Rev. Carrie L. Buckner, 2500 Merced Street, San Leandro, CA 94577, Office Phone: 510-454-4718, Carrie.L.Buckner@kp.org.
• Applications may be sent by email, regular mail, or hand delivered but application fee must be mailed or hand delivered to supervisor.
• Once application is reviewed, student will either be invited for interview or informed that they will not be considered. This generally happens within a week of receiving application if it is within the application timeframe – six months to two months before unit start date.
• Following interview, student will be informed of their acceptance or denial within a week of the interview.
• Students have one week from acceptance to inform the center of their choice to accept or decline the offer and pay the acceptance fee.
