Member Services

Member Services provides assistance with benefit and coverage interpretation, co-payments and premium information, and enrollment information and eligibility including Personal Advantage, Senior Advantage, and other state and federal Programs. We also help members with name or address changes, as well as with the replacement of lost or stolen health plan ID cards. In addition, Member Services assists members with information on the dispute resolution process and evidence of coverage booklets. Members Services is a walk-in-center only.

Our Release of Medical Information department is temporarily virtual. Call 559-448-4938, email Fresno-ROI-Dept-Fax@kp.org or visit kp.org/requestrecords to request and view your medical records; file a disability claim or extension; or request Family and Medical Leave Act (FMLA) certification.
