Kaiser Permanente East Bay HouseCalls
Every month, Kaiser Permanente physicians and other clinicians from the East Bay host an online patient centered broadcast called East Bay HouseCalls. This is a virtual communication platform created for any Kaiser Permanente community experiencing a gap in health care. The program allows members to learn how to prevent and manage health conditions and ask questions during recorded sessions. Our goal is to S.E.E.E. our members and create conversation around making healthy lifestyle choices, self- advocacy, and best practices in the prevention of chronic disease.
- Support our members and community to take control of their preventive care and chronic conditions
- Explore and identify opportunities to improve care
- Encourage dialogue and partnership between members and medical specialists
- Educate on how to care for chronic conditions at home
