Young Adult Transition Program

Kaiser Permanente’s Pediatric Endocrinology and Pediatric Hematology Departments at the Oakland Medical Center are committed to helping our patients make a smooth transition from pediatric to adult health care. To do so, we have developed a program called the Young Adult Transition Program  (YATP).

This program involves working with young adults age 17+ and their families to prepare for the change:

  • from a PEDIATRIC MODEL OF CARE – parents make most of the decisions
  • to an ADULT MODEL OF CARE – young adults take on responsibility for decision making

Through in-person and/or phone appointments with a Program Coordinator, we hope to help you establish care with your Adult Primary Care Provider and Adult Subspecialty Doctor. Additionally, with assistance from our Program Coordinator, we would like to support you in the developing skills required to independently:

  • Access your own health record and healthcare services through KP.org
  • Keep track of, make, and cancel appointments
  • Access pharmacy and handle all medication refills
  • Verbalize your health needs and what to do in an emergency
  • Keep up-to-date on necessary vaccinations, office visits, lab work, yearly exams, and more
  • Maintain insurance coverage and/or know who to contact for assistance

At age 18, youth legally become adults. We respect that many of our young adult patients choose to continue to involve their families in health care decisions. Only with the young adult’s consent will we be able to discuss any personal health information with family members. If the young adult has a condition that prevents him or her from making health care decisions, we encourage parents and caregivers to consider options for supported decision making.
