Kaiser Permanente Santa Clara Department of Acupuncture currently offers treatments to patients with a diagnosis of Chronic Pain or for relief of Nausea/vomiting associated with chemotherapy.

Acupuncturists believe that illness occurs when something blocks or unbalances your Qi. Acupuncture is a way to unblock or influence Qi and help it flow back into balance.

Acupuncture is done by inserting very thin sterile acupuncture needles into the skin/muscle at certain points on your body. This is done to influence the energy flow.

Acupuncture is one procedure offered for patients experiencing chronic pain. Long-term chronic pain may be resistant to many traditional medical treatments and can be physically and mentally debilitating. If you are referred to Physical Medicine and Rehabilitation, Acupuncture is one procedure you and your provider here may consider. Advanced Heart Failure / Heart Transplant

We offer advanced heart failure consultation and management, cardiac transplant evaluation, post-transplant management, left ventricular assist device evaluation and access to a variety of clinical research trials. Following intensive consultation, education and evaluation, the majority of heart failure patients referred to our program, ‘graduate’ back to the care of their local general cardiologist and primary care provider once their symptoms are optimally controlled. We provide ongoing continuity care for patients on the heart transplant waiting list and cardiac transplant recipients.

We treat your heart problem as well as help you and your family understand and cope with your condition in the best way possible for you. Santa Clara Breast Care Center is part of the American College of Surgeon’s Commission on Cancer Accreditation Program. This accreditation reflects high quality, multidisciplinary care offered to breast cancer patients.

The Breast Care Center is dedicated to treating patients with breast problems. Such issues may include breast lumps, pain, infection, nipple discharge, abnormal mammograms and cancer. Cardiac Catherization Lab

Cardiac Catheterization (CCL) Lab is an invasive, diagnostic specialty procedure department which performs high quality diagnostic cineangiography and hemodynamic physiologic studies on the adult and geriatric patients. Cardiac Catheterization is used to view and treat potential artery blockages that could cause heart attacks or other serious cardiovascular damage. The Cath Lab also offers Percutaneous Coronary Interventions (PCIs) e.g. Angioplasty – a non-surgical procedure to open blocked heart arteries.

Kaiser Permanente Santa Clara has three Cardiac Catheterization labs serving members in the South Bay and Greater Southern Alameda Area. The lab features an all-digital cardiovascular imaging system. The system enables cardiologists to view hard-to-see blood vessels as well as the devices used during procedures, such as stents, guide wires and catheters.

Our mission is to provide state-of-the-art consultative, diagnostic, & interventional electrophysiology services for Kaiser Permanente members who are referred for management of cardiac arrhythmias.

We educate patients and physicians about cardiac arrhythmias including Slow Heart Rhythms, Supraventricular Tachyarrhythmias, Atrial Flutter & Tachycardia, Atrial Fibrillation, and Ventricular Tachyarrhythmias.

We specialize in the invasive evaluation and treatment of problematic or life-threatening cardiac arrhythmias. We offer an array of services designed to manage your health care needs across the continuum from inpatient to outpatient. Coordination of care involves Home Health and Hospice, Skilled Nursing Facility, Durable Medical Equipment, Ambulance, Case Management, Bed Management and Contract Oversight. The goal of Continuing Care is to provide a seamless transition for you as you move across the continuum.

Patient care coordinators work with patients & their families to help patients make the transition from hospital to skilled nursing facility & from skilled nursing facility to home. Home Health Care

Home Health services for skilled intermittent care that is medically necessary and requires a physician’s order for care. The care includes registered nursing, physical therapy, occupational therapy, medical social worker and Home health aide services in the home. The Department of Hospital Medicine is a team of Physicians within the Department of Internal Medicine who specialize in taking care of hospitalized patients. If you are hospitalized, it is likely that you will be under the direct care of Hospital Medicine physician or be seen by one in consultation during your hospitalization or visit to the Emergency Department. If necessary during the course of your hospitalization, we work closely with your primary physician and sub-specialist physicians ( such as Cardiology, Nephrology and Oncology) to coordinate and direct your overall care. We also work as a team and you maybe seen by different members of our group at different times during your hospital course. Since we provide 24 hour coverage in the hospital, one member of our group may admit you to the hospital from the Emergency Department and then another member may take over your care in the morning. Aside from seeing different members of our group, you may also see resident physicians and medical students still in training. These bright and enthusiastic team members are always under the supervision of a staff Physician and are part of the larger team dedicated to your care.

We know that being hospitalized can be stressful for you and your family. Memory Center

The Memory Center is an interdisciplinary program designed to help provide Santa Clara Kaiser Permanente members with a comprehensive assessment of their memory problems and link them with needed resources.

Patients referred for a full dementia work-up are first invited to attend an initial group session. Normal age-related changes and causes of memory problems are discussed during the group visit and resources are shared with patients and caregivers.

A one-on-one diagnostic evaluation is scheduled following the group visit. The results of this evaluation are reviewed by the Memory Center interdisciplinary team, which includes a geriatrician. Then, a diagnosis and its implications are discussed with the patient and the caregivers.

Neonatal Intensive Care Unit (NICU)

The Neonatal Intensive Care Unit (NICU) provides Family Centered Care to newborn infants that need Intensive care. Our patients include premature infants, infants needing surgery in the neonatal period, or any infant requiring close observation. The NICU staff is highly trained to work with infants and families. We strongly encourage parent participation as our main goal is to integrate a healthy baby into the family unit.

Visiting Hours
Our visiting policy encourages frequent visits by the parents. The NICU is open to parents any time, day or night. There may be short periods of time during the day when visitors are not allowed, such as during report time and doctor rounds (to maintain confidentiality of our patients). Visiting is limited to two people at a time per family. One of the visitors must be banded. Siblings of the patient are the only children under age fourteen that are allowed to visit. The Occupational Medicine Department at Kaiser Permanente – treats both Kaiser Permanente members and non-members for a wide variety of work-related injuries and illnesses. Employees seeking our care for a work-related injury, should check with their employer representative to verify that Kaiser Permanente is an approved industrial injury provider for your company. The important difference is your benefits under the Workers Compensation system that covers work-related injuries and your personal health insurance plan, which may or may not be Kaiser Permanente.

If your personal health insurance is with Kaiser Permanente, you should know that Kaiser Health Plan benefits do not cover work-related injuries. These industrial injuries are treated by a company’s designated health care provider, which may be Kaiser Permanente, but this needs to be verified before making an appointment. Of course, if you are experiencing an emergency, then you should seek treatment at the nearest hospital or Emergency Department.

Wound Care Clinic

The wound care clinic was created out of the need for our patients to have a place where they can get the most comprehensive, accessible and knowledgeable care of essentially any type of wound that your physician feels is appropriate as out patient care.

The clinic was initially created as an extension of the Department of Surgery in 1997, but now has expanded its role and now gets referrals from all over the medical center and is run by a very qualified group of wound care nurses who are always supported by the staff surgeons. The clinic sees about 3500 new patients every year, and has a large variety of wound care supplies available to accurately take care of nearly any type of wound that is encountered.

Medicare Members: To request a hard copy of Kaiser Permanente’s provider directory, please call our Member Services department at 1-800-443-0815, seven days a week, 8 a.m. to 8 p.m. Kaiser Permanente will mail a hard copy of the provider directory to you within three (3) business days of your request. Kaiser Permanente may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the provider directory in hard copy permanently.

If you request it, your request for hard copies of the provider directory remains until you leave Kaiser Permanente or request that hard copies be discontinued.

Medi-Cal Members: This directory includes Kaiser Permanente’s outpatient pharmacies. You can get any drugs covered by Kaiser Permanente at one of these pharmacies. You can get outpatient drugs covered by Medi-Cal at any Medi-Cal Rx Pharmacy. It does not have to be a Kaiser Permanente network pharmacy.  Most Kaiser Permanente network pharmacies are Medi-Cal Rx pharmacies. Your pharmacy can tell you if it is part of the Medi-Cal Rx network. If you want to find a Medi-Cal pharmacy outside of Kaiser Permanente, you can use the Medi-Cal Rx Pharmacy Locator online at www.Medi-CalRx.dhcs.ca.gov. You can also call Medi-Cal Rx Customer Service at 1-800-977-2273, 24 hours a day, 7 days a week (TTY 711 Monday through Friday, 8 a.m. to 5 p.m.).

Medi-Cal Members: Enhanced Care Management and Community Supports services require prior authorization and are limited to members who meet specific eligibility criteria.
