Patients with serious illness and their families live as well and as fully as possible.

We promise our members and their families that we will:

  • Offer them palliative care.
  • Learn about their beliefs, values and goals.
  • Keep them and their goals at the center of their care.
  • Share their plans with all who care for them.
  • Honor their wishes.

Palliative Care is a trans-disciplinary medical specialty made up of a team of doctors, nurses, social workers, chaplains and sometimes pharmacists. This team works together with a patient’s primary care team to provide an extra layer of support for our patients and their families who are facing a serious illness.
